The Kansas Communities That Care Survey is administered to students in grades 6, 8, 10, and 12. This anonymous survey provides insights on the health risk behaviors and opinions of students in Kansas.

Parents must provide consent for students to take this survey. Parents may give permission using the English form here or the Spanish form here.

Survey questions can be found here. USD 507 will administer the Core Survey and the Depression Suicide modules only.
